Summary:

The long-term objective of this project is the development of a practical, theory-based nursing intervention that will increase medication adherence and improve the clinical effectiveness of highly active antiretroviral therapies (HAART). The specific aims of the study are:

  1. To determine whether a Freirian, home-based nursing intervention improves adherence to HAART as measured by medication event monitoring system (MEMS) data, self-report, and unannounced pill counts compared to standard care.
  2. To describe the relationship between the home-based nursing intervention and biologic measures of clinical outcome, specifically, plasma quantitative HIV RNA levels and CD4 lymphocyte counts.
  3. To describe the relationship between the home-based nursing intervention and a subjective measure of clinical outcome, specifically, subjects' perception of health status.
  4. To explore the relationship between the home-based nursing intervention and contextual factors related to adherence, specifically social support, trust in provider, and knowledge of the purpose of HAART.
